Prevention of the next outbreak of SARS

Please read the forwarded message very carefully. It is from Dr. Justin Wu - Doctor in ward 8A HKG,who has been managing the SARS in Prince of Wales Hospital for the past few weeks.

Dear all,

Hundreds of staff are working with me and they are all facing the same risk of infection. It is really sad that over last 2-week fight 8% of our staff have been infected despite droplet precaution, probably due to pitfalls like air leak on N95 mask, rubbing eyes/nose after touching the mask, etc. So you can imagine how contagious this virus is! The outbreak in Amoy Garden is not surprising to me because this is exactly what has happened in ward 8A. The only difference is that the number of infectious index cases now in Amoy Garden is 60 times of PWH's.

We do believe this infection has wide spectrum of clinical presentation ranging from sore-throat and low grade fever to full blown ARDS/BOOP. We think the clinical presentation depends partly on the dose of inoculum. The ultimate protection against this new virus is our immunity and inevitably all of us will be infected eventually. The purpose of preventive measure is to minimize the dose of virus so as to avoid severe complication.

Could you please forward these messages to all your friends and relatives and ask them to distribute further?

1. Always wear a well-fitted mask (N95 or 3 ply surgical) in escalator, bus, MTR, train, office, restaurant and shopping mall. Don't go to cinema. Push the button in the lift using your key to avoid any direct contact with your bare hand. If you can tolerate a latex glove, wear it.

2. Don't touch your mask, it traps a lot of droplets.

3. All the surfaces within a distance of 5 feet from a patient are highly infectious because of droplets. Wash your hands frequently. Don't rub your eyes, touch your nose or mouth before washing hands.

4. Clean the door handles, furniture and floor with diluted (1 in 100) bleaching detergent. Dettol is NOT useful. Wash your clothes immediately after going home.

5. Patient is highly infectious 2-3 days (in contrast to what EK Yeoh said) even before developing illness. Treat those without wearing mask potentially infectious even if they look healthy.

Take care buddy.

The attached message is from the Chief Executive of Queen Mary Hospital regarding SARS

Weekly Report from CCE of HKW on SARS and

Guidelines for Individuals & Family Members of the HKW Cluster Hospitals

26 March 2003

Dear Colleagues,

It has been two weeks since the first outbreak of SARS in PWH. Up to today there are 290 cases reported throughout Hong Kong, including 117 medical and health workers and 17 medical students. Four staff from GH and TWH have volunteered to help out in PWH, the hospital with the highest number of patients and infected staff. Their compassion and selflessness is appreciated by all.

We are observing a linear increase of about 20-25 cases reported each day, with evidence of sporadic emergence from the community, plus second level or third level infections from the original index cases. From the preliminary investigations and experience of the SARS outbreak, and the remarkable discovery of our microbiologists, it is evident that the infection is mostly likely to be caused by a new virus similar to the common cold virus. Instead of producing mild upper respiratory symptoms, it affects the lung tissues rapidly, resulting in pneumonia of the lower lobes of the lung. The infection spreads by droplet and contact, but not airborne. The incubation period ranges from 2-7 days, and most commonly 4 days. The prominent symptoms are high fever (38oC), muscle aches, chills and rigor (shivering). Cough, running nose and sore throats are relatively uncommon. X-Ray chest of patients with obvious symptoms would show early changes of pneumonia. CT scan of the lung is indicated for doubtful cases. Patients with unresolved prominent symptoms and negative X-Ray should be followed-up daily with repeat X-Rays. Some patients might also have mild gastrointestinal symptoms. With recently accumulated experience, most patients respond well to a combination of high-dose steroid, Ribavirin and cover with board-spectrum antibiotics. Patients with diagnosis with SARS should be segregated for at least three weeks after the onset of symptoms, in order to ensure no further shredding of the virus which might cause infection to family members.

Guidelines for protection of staff, patients and visitors in hospitals have been given and updated earlier, including cleansing instructions for our wards, offices and common areas in hospitals. As there are already signs of emergence of SARS from the community, through secondary and tertiary infection of hospital staff and their family members, we have to exercise great care to ensure also the protection of our family members, in addition to patients and staff, and contribute to the limitation of spread into our community.

With the expert input of our microbiologists of HKU/QMH, we have developed the following recommendations and guidelines for our family members. They should be applied and considered together with the other guidelines from HA and Department of Health. We need to exercise our common sense according to our home environment and make judgement according to our needs. Panic reaction would diminish our wisdom and often lead to irrational decisions and over-reactions. We shall continue to update on those guidelines when there are new information available. Take good care of yourselves, your families, so that we can serve the community effectively.

With Best Wishes and May God Bless Us All,

Dr. York CHOW

Cluster Chief Executive

Hong Kong West

Guidelines for Protection of Individuals and Family Members

(from Anti-SARS Task Force of HKW Cluster)

Principles in controlling spread of droplet/contact infection

  1. To isolate/segregate infectious and potentially infectious individuals.
  2. To minimize close human contact and unnecessary human contact (including reduction of unnecessary human traffic).
  3. Self protection and protection of family members:

Three simple steps:

Protective mask

Hand Washing

Self-imposed segregation

Guidelines for Protection of Individuals and Family Members

I. For everyone, adults and children: -

  1. Avoid going to areas with too may people. Wear a N-95 or surgical mask if going to cinemas, theatres and concerts. (Surgical mask is more comfortable but must be disposed of when wet. N-95 must be worn properly without any leak. Single layered paper masks are not effective.)
  2. Avoid restaurants that are too crowded. Do not share tables with strangers.
  3. Avoid unnecessary travelling to Shatin, Tseung Kwan O and their neighbouring areas.
  4. Keep a mask with you and wear it in crowded places such as public transport vehicles and area full of crowds (shopping malls/markets).
  5. Avoid kissing and minimize shaking hands in social encounters and activities.
  6. Wash hands with soap regularly after touching any objects suspected to be contaminated. Wash hands before and after each meal, and avoid touching your eyes, nose and lips with unwashed hand/fingers.
  7. Children and students should wear masks to schools, but must ensure they do not touch the external surface of the masks, and wash hands after touching them.
  8. Parents of young children (play-groups, kindergartens and P.1-3) can consider keeping them at home for 2 weeks (or further if necessary). Outings should be kept to a minimum with observation of the previous guidelines.

II. For individuals with suspected symptoms:

  1. High fever, muscle aches and chills/shivering are symptoms of significance, and doctors should be consulted if they last more than 24 hours.
  2. Go to the most nearby doctor of your choice, and wear a mask at all times. If SARS is diagnosed, you will be transferred to the designated HA hospital. Also make sure people who accompany or transport you are wearing masks.
  3. Take X-Ray chest and blood tests as recommended by the doctor, and also take repeated X-Rays as instructed.
  4. If you are suspected to have contracted SARS, your family members and accompanying persons must also wear masks when they are close to you.
  5. Cancel all social engagements and rest at home if SARS cannot be totally ruled out by your doctor.
  6. Make a note of your activities in the past seven days, and the people that got close to you that might have been infected. Inform your doctor accordingly.
  7. Keep a list of your close encounters after the onset of symptoms, and inform your doctor if the SARS diagnosis were made.

Although some of the above measures might be considered excessive by some, they are nevertheless easy to comply and be supervised. As we are facing an unknown but potentially deadly virus with an unpredictable outcome, we prefer to be over-protective rather than complacent at this very critical time.

How to wear your mask

First of all, before you get all happy about being able to get a hold of
a mask, please learn how to wear one. If you don't, your case would be
worse than people without a mask. If, after you have a mask on, you
still have leaks at the two sides of your cheek, then you are obviously not wearing it right. The problem is, if virus or germs leaks in, then you'll be breathing that virus or germs all day long. Freaky, isn't it?

Every mask is wore on differently. However, most mask (N95, 8210,
surgical masks...or similar) should be able to wrap around your chin!

YES! THE CHIN!

Or else you'll definitely find a leak at the two sides of your cheek!
As everyone should know by now, N95 has the biggest protection. It blocks
both big and small droplets. BUT (yes...but), it also has the worst ventilation! If anyone has tried to wear for a day, they'll probably know.

Next best thing are surgical masks, which you'll probably see two kinds
in the market, one with the ear loop (which are usually call Procedure
mask), and one with two ropes which you are suppose to tie yourself. Of the two, the latter works better.

Surgical mask - Way to put it on:

1. With the green side facing out (or which means the white side towards yourself), and with the wire on the topside (meaning, on your nose), press along the wire to adjust/mold it to your face's and nose's shape.

2a. With the ear loop, just loop it around the ear! Pull to spread the folds until it has wrapped around the chin. done!

2b. With the one with ropes (the better one), you wrap the upper rope around your head (just above the ear). Tie a knot.

 3. As you could see the folds on the mask, you pull the mask downwards, wrapping the chin.

4. The lower rope should now be near your jaw. Wrap it upwards toward the top of your head. Tie a knot at the top of our head. (Yes, you should look like a lovely wrapped gift by now.)

5. Check for leaks on two sides of your cheeks. Your are safe!

LOOKING LIKE AN IDIOT FOR A COUPLE WEEKS IS BETTER THAN RISKING YOUR LIVES! YOU ONLY LIVE ONCE!

 Second precaution: WASH YOUR HANDS!

Hopefully, you have disinfectants. If not, at least use some soap.

 1. Rub your palms back and forth (try 5 times).
 2. Rub one palm against the other hand's back. (5 times). Switch hands.
 3. Wrap right hand around left thumb. Twist. (5 times). Switch hands.
4. At the same time, put your 4 fingers in the palm of the other hand, and twist. Kind of like tying your fingers to your palms, knotting your hands together. (or, like the position of the hands of an opera singer)
 5. When rinsing your hands, rinse it with your fingers pointing downwards to the drainage so that germs will be flushed away from your hands.

Right now, since we don't know much about the virus, try to keep yourself as clean as possible. If you must go to a hospital, wash your hands before and after you leave the hospital.

Also, especially for GIRLS:

 1. Try not to keep your nails too long, as it does catch a lot of germs and bacteria.
 2. Now is the time to catch up all your beauty sleep so that you'll save enough energy to fight any contamination, viruses, germs, and bacteria.
3. SHOWER at NIGHT. We can't be sure now as to how long the virus could live before it finds a host. Some say 6 hours. So, showering at night gives you the benefit of not bringing it to bed and then breathing it in yourself again. ESPECIALLY those girls (well...guys now too) who have long hair.

For example, say if someone has sneezed or coughed and you caught some on your hair. If you go to bed with that virus on you, you'll catch it easily with just a breeze or smelling your hair (as I know many girls (somehow) love  to smell their shampoo off their hair).

 For everyone:

1. When travelling on public transit (bus, mtr, trains, vans, and taxi), remember not to touch yourself (especially your eye (lid), rubbing your nose, handling food, or even sticking your finger into your ear). As this virus is passed through any part of your body which has a membrane (meaning, it's permeable to membranes with mucus). Well, forget about the science, just bodily secretion from any part of your body which has mucus (eye, nose,
ear, mouth, sex organ). In another word, after getting off public transit or other places (such as rails of escalators, stairs, etc), remember to WASH YOUR HANDS before rubbing yourself.

 2. Don't go to hospital unless your have to. And if you have to, keep your visit short.

 3. If you have symptoms of atypical pneumonia, go to the doctor ASAP before you pass it to others. Briefly, some symptoms are: flu, fever of 38 degrees celsius or higher, shortage of breath, feeling periods of hot and cold, and increasing or a new cough. (I'll try to include an more accurate and extensive list tomorrow.)

4. Please go to the doctor if you feel you might have it or else it'll be in the community forever!! Earlier diagnosis could greatly increase your chance of living and greatly decrease your chance of spreading it. Save your life and others!
